The following is the statement that was included in the email that we received from Ms. Roslyn Thompson on October 7, 2005:

Dear colleagues:

Attached is the Rehabilitation Services Administration (RSA)'s notification that a revised 704 Report will be in effect for the FY 2006 data collection. The reporting instrument and instructions for Parts I and II of the revised 704 Report are attached as well.

The attached instruments and instructions are substantially identical to the versions published in the Federal Register on September 12, 2005, except for one slight modification, namely, in subpart II the order in which the Race and Ethnicity questions appear was changed. Ethnicity (section G) now appears before Race (section H).

The revised 704 Report has been approved for collecting FY 2006 data only. For FY 2005 data, please use the reporting instrument and instructions emailed to you on November 5, 2004. These have not been modified and remain in effect for FFY 2005 reporting purposes.

P.S. The 704 Report revision is based on new performance measures established for the IL program based on a Program Assessment Rating Tool (PART) evaluation conducted by the Office of Management and Budget. A web link to the new measures is provided below, for your convenience.
