The following is the statement that was included in the email that we received from Ms. Thompson on October 27, 2005:

Attached are the 704 Report Instrument and Instructions that is to be used for 2004-2005 Reporting year. The new revised 704 Report that I sent out on October 12, is to be used for the 2005-2006 Reporting year only. It is very important that I receive your 704 Report by the due dates. 722 States - December 31, 2005 and 723 States January 31, 2006. Thank you
