Transitions Facilitator Description

Employment Type: Full Time
Location: Berkeley, CA
Salary: $31 – $32.22 per hour

We seek a visionary collaborator with a justice-oriented approach to work with individuals with disabilities on transition plans from institutional settings, including jails, prisons, nursing homes, and homelessness. Responsibilities encompass holistic transition planning and establishing collaborative relationships with community-based organizations, public defenders' offices, juvenile justice systems, homeless shelters, and others to support people with disabilities in community living.
