The 2023 USAging Business Innovation Awardees

The Aging and Disability Business Institute announced the winners of The John A. Hartford Foundation 2023 Business Innovation Award, highlighting the successes of community-based organizations that partner with health care entities to innovatively promote age-friendly care.

Winner: Western New York Integrated Care Collaborative, Inc.
WNYICC’s 50+ members help reduce barriers older adults and people with disabilities may face accessing medical care by addressing social needs like food insecurity, social isolation, health literacy and more.
Runner-Up: Central Ohio Area Agency On Aging
The organization’s Social Determinant of Health Accelerator Program allows community health workers, social workers and registered nurses to screen patients for health-related social needs in Columbus, OH.  
Runner-Up: Region IV Area Agency on Aging

Through its partnership with Corewell Health South and a network of community-based organizations in southwest Michigan, the organization reduces the frequency of emergency department visits by older adults through the 4Ms framework.
