Central Ohio Area Agency on Aging (COAAA) plans, funds and delivers services to over 40,000 older adults and people with disabilities in an eight-county area. The partnership between Central Ohio AAA and Molina Healthcare of Ohio helped create COAAA’s Social Determinant of Health Accelerator Program, a social care model focused on improving health equity, workforce development and social determinants of health (SDOH) interventions.
