Amanda Strantz started with The Independence Center as the Care Transition Coordinator for the Hospital to Home Pilot Program in 2018. As the Care Transition Coordinator, Mandi provides intensive case management services to patients discharging from the hospital back into their homes. Before joining The Independence Center, Mandi was a child welfare caseworker with DHS and worked as an intake coordinator with The Resource Exchange, the local single entry point for long-term services and supports. She holds a Bachelor of Science in psychology from Southern Illinois University at Edwardsville.
