The U.S. Administration for Community Living invites you to join fellow community-based organizations, hospitals and health systems for a peer-based learning webinar series. The monthly meetings will leverage participants’ expertise to develop collaborative approaches to health-related social needs screening within health care settings, connection to community services and supports and transition support pre and post-discharge for older adults and people with disabilities. The series will run from January through August 2024.
